ABOUT THE SERVICE: One of the mandates of the Municipal Health Office is to diagnose and treat illnesses and give appropriate medical intervention and/or professional medical advice. Service is available to any person/individual who needs medical assistance.

ABOUT THE SERVICE: Medical Certificates, Health Certificates and Medico-Legal Certificates are prerequisites of some agencies. They are generally required in getting drivers licenses, enrolling in schools, getting a visa, employment in food establishments and in filing cases in public courts.

ABOUT THE SERVICE: Dental care is being offered to the community to ensure proper and adequate oral hygiene. The preventive aspects are done in different BHS while the curative care is done at the main health center.

ABOUT THE SERVICE: This program is being implemented to treat patients diagnosed of having an active Pulmonary Tuberculosis, arrest the spread of the disease prevent the development of Multi Drug Resistant TB.

- ISSUANCE OF DEATH CERTIFICATES, PERMIT TO TRANFER CADAVER, SANITARY PERMITS
ABOUT THE SERVICE: These services are being provided to declare a person to be legally dead, determine his/her cause of death and determine if the cadaver can be transferred from this municipality to other towns or cities. Sanitary permits are being issued to business and food establishment after proper inspection. It will be released after the Sanitary Inspector conducts actual inspection and determines that no violations of the sanitary code of the Philippines and other local ordinances are being committed.

I. FOR DEATH CERTIFICATES AND PERMIT TO TRANSFER CADAVER
1. Go to the Local Civil Registrar and register the name of your dead relative. 2. After the forms are filled up proceed to the Municipal Health Office. 3. You will be interviewed regarding the events and circumstances leading to the death of your relative. Please provide necessary information. 4. After the cause of death is determined, the Municipal Health Officer will fill up and sign the death certificate form. 5. If permit to transfer of cadaver is needed, the Rural Sanitary Inspector will fill up and sign the PTC form and submit it to the MHO for approval 6. You will be ask to pay the necessary fee for the death certificates and IF needed the fee for the permit to transfer of cadaver.

II. FOR ISSUANCE OF SANITARY PERMITS | |||
a. Go to the Municipal Treasurers Office and file for the application of sanitary permit.
b. Proceed to the Municipal Health Office. You will be instructed by the RSI on what to do:
1. For Non Food Establishments
– Filling up sanitary permit forms – Schedule of Inspection – Inspection Report and recommendations
2. For Food Establishments
– A laboratory examination of urine, sputum and stool will be conducted for you and your food handlers.
– Directly Observed Deworming treatment of all your food handlers

VIII. PROVISION OF EXTENTSION SERVICES TO DIFFERENT BARANGAYS THRU THE BARANGAY HEALTH STATIONS AND THEIR CATCHMENT AREAS
ABOUT THE SERVICE: Implementation of the preventive and promotive programs of the LGU and the DOH begins at the level of the BHS. Micronutrient supplementations, tetanus toxiod immunizations and dental care are given to mothers during pregnancy to ensure that a healthy baby will be born to this world. After Birth, children are being provided with Immunizations to protect them from contacting preventable diseases such as measles, polio, diphtheria, tetanus, pertusis and hepatitis B. They are also being given with Micronutrient supplementation and Deworming treatment to prevent micronutrient deficiency and treat intestinal parasitism. The community received assistance for securing safe water and toilet constructions to prevent food and water borne diseases. BHS Services are also aimed to bring health services closer to the community.

4. If you need PRENATAL CHECK UP, you will be interviewed initially by our BHW. Please provide the necessary data. If it is the first time that you consulted with your RHM, your name will be recorded in the TCL and the masterlist of the Women’s Health Team (WHT) for the pregnant women in your barangay.

a. Basic anatomy and Physiology of the reproductive tract. b. What is reproductive health care? c. The different reproductive care methods of birth spacing and control. d. Benefits of different methods and its possible complication and side effects. It is you who will choose which method is suitable to you and your husband. We are implementing the cost recovery scheme for our reproductive health care commodities. You will be asked by our RHM to reimburse them of the commodities that you choose. If you choose the permanent method of birth control, our RHM will refer you to our MHO for proper counseling screening and scheduling of the procedures.
